Prescription savings for Medicare Patients
San Jose Medical Group recently began offering a program that helps provide prescription medications
savings for eligible patients. The Together Rx Card, sponsored by seven major pharmaceutical companies,
provides Medicare-eligible enrollees with savings on more than 150
widely prescribed medicines.
The card is designed to save patients 20 percent to 40 percent or more off the current price of medications.
Because each participating member
company sets its own discounts independently, actual savings across products will vary,
Eligible Medicare patients must meet
income criteria and be without current public or private prescription drug coverage.
San Jose Medical Group is encouraging its eligible patients to enroll in Together Rx to
help to provide savings on medications for those who need it most.
Patients can visit their San Jose medical Group physician or call
800-865-7211 between 9 a.m and 9 p.m EST Monday through Friday to answer questions
and receive an application for the together Rx Card Program.

Prescription Refills
To renew or refill a prescription, call the pharmacy that issued your prescription
(the phone number is on your prescription label).
Identify the prescription number on your medication bottle or packet.
Your pharmacy will refill your prescription if refill authorizations remain.
If no refills remain, your pharmacy will contact your physician for additional
authorization to refill the medication.
Call the pharmacy at least three business days before your
medication refill is needed.
Prescription Transfer
If you want to transfer your prescriptions to a different pharmacy,
you should call the new pharmacy and read the prescription label
on the medication bottle or packet to the pharmacist.
The new pharmacy will call your old pharmacy to transfer any remaining refills.
Prescriptions that have no additional refills to
transfer will be called to your physician for authorization.
If your prescription is old and the directions or dose has changed,
be sure to inform the new pharmacist of these changes.
New Prescriptions
If you were given a trial sample of a new medication by your physician
and you wish to continue this medication, contact your physician's office.
When requesting a new prescription, have the sample medication bottle or
packet with you so that you can communicate the necessary information
to the nurses.
Please have the name and phone number of the pharmacy where you
would like the prescription available.
