We at the San Jose Medical Group are aware of our patients' concerns about privacy.
We take utmost care to make sure that all your medical and other information
is kept confidential and used in a manner that is in accordance with regulations
specified by HIPPA - The Health Insurance Portability and Accountability Act.

Section 1300.67.8 of Title 28 of the California Code of Regulations requires that all
health care service providers who contract with health care service plans
(HMOs and some PPOs)
prominently display a specified notice informing patients of how to contact
their health plan,file a complaint with their plan,
obtain assistance from the Department of Managed Health Care
and seek an independent medical review.
